I had a flare and had to go up to 15mg pred 2 months ago from 4.5. I was pretty much pain free. Now, as I taper, the pain is coming back in my feet and my wrists this time, not in my shoulders or arms where it was when I was first diagnosed. Do you think this is still PMR?

Why di yiu need to go as high as 15mg? … usual advice for flare is a short time on an extra 5mg above the dose you last felt okay at.
What dose did the pains in feet/wrists occur? Both can be affected by PMR - as you can see in related posts.
